Brightwater, a respected not-for-profit organisation based in Western Australia, is currently seeking an experienced Admissions and Discharges Coordinator to join our dedicated team.

This role is part of our Transition Care Program (TCP), which is designed to optimise clients’ health, wellbeing, and independence, ensuring a smooth transition from hospital to home or residential aged care.

The opportunity

This is a full-time position, working Monday to Friday during core business hours, based at Brightwater Kingsley. You will support a 41-bed transition care facility, coordinating admissions and discharges to ensure seamless client experiences.

This is a rewarding opportunity for a Registered Nurse with a strong background in complex clinical care, exceptional communication skills, and a passion for driving excellence in transitional care.

The Role:

The primary role is to liaise with the Department of Health WA (The Hub) and the site multidisciplinary team to coordinate admissions and discharges for TCP, ensuring clients meet the admissions criteria as per TCP guidelines, in consultation with the Service Manager, in accordance with the:

  • Timely management of transition care bed movements to ensure maximum occupancy is maintained and compliance from a discharge point of view.
  • Evaluate referral documentation including ACCRs and TCP referrals for all potential TCP clients to ensure clients meet TCP guidelines.
  • Provide site staff with relevant details of admission, ensuring they are aware of needs prior to admission, including notifying the administration assistant.
  • Ensure tasks allocated to the Admissions & Discharge Coordinator on the TCP Admissions Checklist and TCP Discharge checklist are complete.
  • Participate in TCP Quality audits, perform Registered Nurse tasks as required.

  • Proven experience in coordinating admissions and discharges within a healthcare setting
  • Strong clinical expertise, particularly in complex care
  • Excellent interpersonal and communication skills
  • Ability to work collaboratively with multidisciplinary teams
  • A proactive and organised approach to managing client transitions
  • Current registration with AHPRA as a Registered Nurse

To be successful in this role you will have:

  • Registered with Australia Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
  • Demonstrated ability to assess and evaluate client care needs within a nursing and allied health framework.
  • Demonstrated understanding of the complex clinical environment within aged care.
  • Excellent communication (both written & verbal) and interpersonal skills that delivers effective leadership, builds and develops teamwork and establishes successful working relationships with stakeholders.
  • Working knowledge of Occupational Safety and Health legislation; Aged Care Act (1997) and/or Disability Services Commission Standards (1988).
  • Recent acute, subacute, restorative or GEM experience in an adult hospital/transitional care setting (desirable).
  • Satisfactory National Police Certificate obtained within the last 12 months.
