Brightwater At home has two exciting opportunities available for a reliable and motivated Occupational Therapist (junior or senior) to join our Brightwater At Home team, working in our Community Aged Care programs in Central Perth Metro and South of the river (down to Mandurah).
We have multiple full-time/part-time positions available, working 3 or more days per week, from Monday to Friday with our clients in the community. You will have access to our new Brightwater Central facility in Inglewood or work from the comfort of your home. Self-rostering with ability to be flexible with how you work your contracted hours.
You will work with a mix of clients receiving either Home Care Package or Transitional Care Community package. Transition care provides short-term care to optimise the functioning and independence of older people after a hospital stay. Transition care is goal-oriented, time-limited and therapy-focused. The programme seeks to enable older people, where possible, to return home after a hospital stay rather than prematurely enter residential aged care.
You will be working as part of a multidisciplinary team delivering the program with support from dedicated client coordinator, physiotherapists, occupational therapists, speech pathologists, dietitians, social worker, massage therapists, podiatry, therapy assistants nurses and community support workers.
