We're on the hunt for a dynamic individual to join our Disability team as a Community Integration Specialist, providing support to our clients transitioning from our Brightwater Oats Street facility into the community.

As our Community Integration Specialist, you will focus on working with individual’s goals and facilitating the plan for their future living options, maintaining autonomy and a good quality of life. This role will see you supporting our residents, clients and or their guardians to identify appropriate accommodations and support to meet their needs in line with self-determined goals with a focus on community integration.

Your roles and responsibilities include but are not limited to: 

  • Providing information, feedback and recommendations working as part of a multidisciplinary team.
  • Coordinating the transition and community integration of discharging clients.
  • Liaising with relevant stakeholders to coordinate client’s final discharge reports.
  • Supporting the client to create and maintain a home and social environment where their needs are being met.
  • Providing regular reports on progress and client flow updates to the Oats Street Team.

To be successful in this role, you will have:

  • Demonstrated experience working with residents in a residential facility or clients in a community setting with complex needs (Experience working with people with an Acquired Brain Injury is highly advantageous)
  • Excellent communication and interpersonal skills.
  • Demonstrated ability to organise and prioritise tasks to meet deadlines.
  • A strong understanding of incident reporting.
  • A current valid Driver’s License as defined by the Department of Transport WA (DoT)
  • Knowledge of NDIS and current community resources is highly desirable.
  • Satisfactory National Police Certificate obtained within the last 12 months.
