An exciting opportunity is available for a reliable and motivated Physiotherapist (junior or senior) to join our Brightwater At Home team working in our Community Aged Care programs. New graduates are encouraged to apply!
This is a permanent part time position offering 60 hours per fortnight (4 days per week), with flexibility in arranging your own schedule from Monday through Friday with our clients in the community! This role will support our clients who are based South of River
Our Brightwater at Home team is based out of our lovely head office in Inglewood with access and flexibility to work out of our various facilities across the Perth Metro, or from the comfort of your home.
You will work with a mix of clients receiving either Support at Home Packages or Transitional Care Community packages. Transition care provides short-term care to optimise the functioning and independence of older people after a hospital stay. Transition care is goal-oriented, time-limited and therapy-focused. The programme seeks to enable older people, where possible, to return home after a hospital stay rather than prematurely enter residential aged care.
You will be working as part of a multidisciplinary team delivering the program with support from dedicated client coordinators, physiotherapists, occupational therapists, speech pathologists, dietitians, social worker, nurses and community support workers.
