An exciting opportunity is available for a reliable and motivated Physiotherapist (junior or senior) to join our Brightwater At Home team working in our Community Aged Care programs.
This is a permanent part time position offering 45 hours per fortnight (3 days per week), with flexibility in arranging your own schedule from Monday through Friday with our clients in the community!
Our Brightwater at Home team are based out of our lovely head office in Inglewood with access and flexibility to work out of our various facilities across the Perth Metro, or from the comfort of your home.
You will work with a mix of clients receiving either Home Care Package or Transitional Care Community package. Transition care provides short-term care to optimise the functioning and independence of older people after a hospital stay. Transition care is goal-oriented, time-limited and therapy-focused. The programme seeks to enable older people, where possible, to return home after a hospital stay rather than prematurely enter residential aged care.
You will be working as part of a multidisciplinary team delivering the program with support from dedicated client coordinator, physiotherapists, occupational therapists, speech pathologists, dietitians, social worker, nurses and community support workers.
