Reg Nurse – Transition Care Program (TCP)

Brightwater Kingsley is a warm and welcoming home offering short-term care for individuals transitioning from hospital to home or to more suitable long-term accommodation. We are currently seeking compassionate and experienced Registered Nurses level 1.4 and above to join our dedicated TCP team.

About the Role:

As a Registered Nurse in our Transition Care Program, you will play a vital role in supporting older adults who are transitioning from hospital to home or residential care. Their duties are both clinical and supportive, focusing on recovery, independence, and quality of life.

The environment is fast paced, acute style with high turnover inclusive of a broad range of clinical presentations. It offers dynamic and clinically engaging environment.

The work is meaningful and rewarding, helping patients transition safely back to their homes or appropriate care settings.

On Offer: Casual shifts working AM, PM and or ND Monday – Sunday, the ideal candidate is available to work 3 weekday shifts.

Key Responsibilities:

  • Comprehensive assessments: Evaluate residents’ physical, emotional, cognitive, and social needs.
  • Medication management: Administer medications, monitor side effects, and ensure compliance with prescriptions.
  • Wound care and chronic disease management: Provide treatment and monitor progress for conditions like diabetes, heart disease, and pressure injuries.
  • Care planning: Develop and update individualized care plans in collaboration with multidisciplinary teams.
  • Monitoring and documentation: Track health status, record observations, and report changes to relevant healthcare providers.
  • Transition coordination: Help residents and families navigate the move from hospital to home or aged care, ensuring continuity of care.
  • Education and advocacy: Educate residents and families about health conditions, medications, and self-care strategies.
  • Team leadership: Supervise and mentor enrolled nurses and care staff, fostering a collaborative care environment.
  • Emotional support: Provide compassionate care and emotional reassurance to residents and their families.

What We’re Looking For

  • Current AHPRA registration as a Registered Nurse
  • Minimum 4 years’ experience in aged care (TCP) acute setting or rehabilitation settings
  • Ability to recognise  and  respond to  clinical deterioration
  • Strong communication and teamwork skills
  • Empathy, patience, and a genuine commitment to client wellbeing
