As the Service Lead – Transitional Care, you will oversee a dedicated team delivering time-sensitive, high-quality care to individuals transitioning from hospital to home. Your role will ensure clients receive the right level of support at the right time while addressing the complexities of transitional care.

The right candidate will possess: 

  • Tertiary qualifications in Social Work, with eligibility for full membership of the Australian Association of Social Workers (AASW) or the Society of Professional Social Workers; OR registered as an Allied Health Professional or Registered Nurse (RN) with the Australian Health Professional Regulation Agency (AHPRA).
  •  Strong understanding of key performance indicators and making data driven improvements.
  • Ability to provide effective administrative and business support, with a keen eye for detail and efficiency.
  • Experience in  allocating resources and coordinating services within budget and time constraints.
  • Interest in or prior exposure to coordination or case management, ideally within aged care or a health-related setting, with a willingness to develop and grow in the role.
  • Excellent computer skills with sound knowledge of Microsoft applications including Outlook, Word, Excel, Teams.
  • Excellent written, verbal, and interpersonal communication skills and the ability to build rapport and establish successful working relationships with stakeholders.
  • Excellent customer service skills and a clear understanding of individual privacy and confidentiality requirements.
  • Demonstrated ability to work with minimal supervision using initiative and critical thinking, as well as the ability to work collaboratively and inclusively as part of a team.
  • Demonstrated ability to be flexible and work on multiple requirements simultaneously, organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines.
  • A current Western Australian ‘C’ Class Driver’s License (essential).
  • Satisfactory National Police Certificate obtained within the last 12 months.
