We have an exciting new position available for a Service Leader - Transition Care Community (TCC) to join our Brightwater at Home (BAH) team on a permanent full-time basis. 

Supporting clients, families and staff, this position leads a team of TCC coordinators and an administration assistant to oversee the daily coordination operations within BAH. The Service Leader oversees the development and implementation of efficient and client-focused care plans and coordinates services for BAH Transition Care staff and clients.

The right candidate will possess: 

  • Tertiary qualification in Social Work and eligible for full membership of the Australian Association of Social Workers or the Society of Professional Social Workers.
  • Proven experience in managing the admission and discharge of clients within a healthcare or social services setting.
  • Strong understanding of key performance indicators and making data driven improvements.
  • Experience in providing effective administrative and business support.
  • Experience in allocating resources and services within budget and required timeframes.
  • Previous experience in a coordination or case management role, ideally in aged care or health related service.
  • Excellent computer skills with sound knowledge of Microsoft applications including Outlook, Word, Excel, Teams.
  • Excellent written, verbal, and interpersonal communication skills and the ability to build rapport and establish successful working relationships with stakeholders.
  • Excellent customer service skills and a clear understanding of individual privacy and confidentiality requirements.
  • Demonstrated ability to work with minimal supervision using initiative and critical thinking, as well as the ability to work collaboratively and inclusively as part of a team.
  • Demonstrated ability to be flexible and work on multiple requirements simultaneously, organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines.
  • A current Western Australian ‘C’ Class Driver’s License (essential).
  • Satisfactory National Police Certificate obtained within the last 12 months.
