The Opportunity

We are looking for an experienced and passionate leader to join our Brightwater At Home team as our Transition Care Community Service Lead.

This is a rewarding leadership opportunity where you'll play a pivotal role in supporting safe, timely and person-centred transitions from hospital to home, while leading a high-performing team of Transition Care Coordinators.

What you’ll bring  

  • A relevant tertiary qualification in Nursing, Allied Health, Health Services, Community Services or a related discipline
  • Leadership and operational management experience within health, aged care or community services
  • Experience managing referrals, admissions, care coordination, discharge planning or transition services
  • Strong people leadership skills with demonstrated success developing and supporting teams
  • Excellent stakeholder engagement and relationship management capabilities
  • Experience monitoring service performance, data and financial outcomes
  • High-level communication, problem-solving and decision-making skills
  • Strong digital literacy across Microsoft Office and client management systems
  • A satisfactory National Police Clearance issued within the past 12 months.
  • A current valid WA driver’s licence.
