We're on the hunt for a dynamic individual to join our Brightwater At Home team as a Clinical Nurse Manager, providing expert clinical advice and oversight and leading advanced clinical care standards in an in-home care environment.

As our Clinical Nurse Manager, you'll be at the forefront of providing exceptional clinical leadership and governance of community home care services across Brightwater at Home (BAH). The position ensures safe, high-quality, person-centred care in clients’ homes, aligns clinical practice to the Strengthened Aged Care Quality Standards and Brightwater’s Clinical Governance Framework, and leads the development and capability of the clinical workforce across BAH.

Your roles and responsibilities include but are not limited to: 

• Leading and maintaining advanced clinical care standards in home care environment.
• Providing clinical supervision, mentoring, and performance review for registered nurses including enrolled nurses and Care Partners.
• Developing, reviewing and implementing clinical policies, guidelines and procedures specifically for BAH in conjunction with the Clinical Excellence policy leads.
• Collaborating with allied health and other disciplines to ensure multidisciplinary reablement strategies are embedded in care planning and service delivery.
• Monitoring and reporting clinical outcomes, adverse events and compliance metrics to initiate continuous improvement plans and drive continuous quality improvement.
• Actively contributing to requirements gathering, analysis/evaluation, design, and selection of clinical systems.
• Leading the Serious Incident Response Scheme (SIRS) for BAH.

To be successful in this role, you will have:

• Relevant tertiary qualification in Nursing with current AHPRA registration.
• Minimum 5 years of senior nursing leadership experience in aged care or community/home care environment.
• Strong knowledge of the Aged Care Act 2024, Strengthened Aged Care Quality Standards, and Australian community aged care service delivery models.
• Demonstrated experience in clinical governance, risk management, workforce development and multidisciplinary care in home/community settings.
• Excellent interpersonal, communication and stakeholder engagement skills.
• Satisfactory National Police Certificate obtained within the last 12 months.
• Valid driver’s licence to drive in Western Australia, as defined by the WA Department of Transport (DoT).
• Valid and satisfactory NDIS Worker Screening Check.
• Experience in reablement, restorative care, or home-based aged-care programs (e.g., HCP/SAH)  and knowledge of bundled funding, program compliance and care package financial management is highly desirable.
