The opportunity

We’re looking for an experienced Service Manager to lead Brightwater Kingsley—a 41‑bed Transition Care Program (TCP) service. This is a hands-on leadership role in a fast-paced, short-stay environment where you’ll partner closely with a multidisciplinary team to help older people recover after hospital, build confidence, and transition safely back home (or into the right longer-term option).

TCP provides short-term accommodation and reablement support for older people directly after discharge from hospital. You’ll ensure comprehensive assessment and goal setting, coordinate the delivery of clinical and allied health interventions, and monitor outcomes against each person’s individual plan.

Success in this role is seeing people regain function and independence—supporting a confident return home with the right services in place, or a smooth transition into permanent residential aged care when needed.

Permanent, full-time role | Monday–Friday (core business hours and on call flexibility over weekends/PH)

What you’ll be responsible for

  • Lead safe, high-quality clinical care and daily operations across the 41-bed TCP service.
  • Ensure compliance with Transition Care Program requirements, funding and reporting obligations.
  • Embed person-centred, goal-directed reablement practice and measurable outcomes.
  • Build a high-performing team through recruitment, coaching, capability development and effective rostering.
  • Partner with the multidisciplinary team (nursing, allied health, medical and support services) to coordinate seamless care and discharge planning.
  • Maintain strong governance and compliance with the Aged Care Quality Standards and relevant legislation.
  • Manage financial performance, monitor budgets and identify efficiency opportunities.
  • Drive continuous improvement, quality activities and readiness for audits/accreditation.

What you’ll bring

  • Proven leadership experience as a Service Manager (or equivalent) in aged care, sub-acute, rehabilitation or transitional care settings.
  • Relevant clinical qualification and AHPRA registration (desirable).
  • Strong working knowledge of accreditation, quality systems and the Aged Care Quality Standards.
  • Confidence leading multidisciplinary teams and building effective relationships with hospitals and external stakeholders.
  • Sound commercial capability—budget management, resource allocation and operational decision-making.
  • A proactive, data-informed approach to quality improvement and change.
  • A genuine commitment to safe, person-centred care and reablement outcomes.
