We are currently seeking applications for a Social Worker to join our rehabilitation team for people aged 18 – 64 years with an acquired brain injury (ABI) on fixed term fulltime basis working 76 hours per fortnight. The contract will run through to September 30 2026.
As a Social Worker in the transitional rehabilitation program, you will be responsible for completing initial social work assessments to understand each client’s goals and support significant others in the client’s life. You will provide ongoing support to clients and key stakeholders in relation to Administration and Guardianship ores including submissions to SAT, financial assessments and budgeting support, and support with legal matters. You will work closely with a Welfare Assistant. As a key member of the multi-disciplinary team, you will contribute towards goal setting and provide discharge and overall case coordination for transition care clients.

The ideal candidate will:
- Membership, or eligibility for membership, with Australian Association of Social Workers (AASW) (active membership preferred)
- Strong commitment to providing safe high-quality care
- Excellent written, verbal and interpersonal communication skills in various environments with a diverse range of people
- Demonstrated experience as a Social Worker to assess, plan, implement and evaluate client care
- Demonstrated ability to organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines
- Working knowledge of occupational safety and health legislation; and where relevant, the Aged Care Act (1997) and/or Disability Services Commission Standards (1988) and the Guardianship and Administration Act (1990)
- Current satisfactory National Police Certificate (obtained within the last 12 months)
- Possess a current Western Australian ‘C’ Class Driver’s License
- Evidence of up-to-date flu vaccination
- Satisfactory NDIS Worker Screening check
